Speech Language Pathologist SLP
Job Description:
The Speech Therapist (Pathologist) directs, supervises, evaluates and provides speech therapy services to Quality Care Rehabilitation Professionals patients as prescribed by the attending physician and for providing Agency staff, and the patient's physician with speech therapy consultation, as needed.
Job Duties:
o Assesses, plans, and implements rehabilitation services and for speech, language, and/or swallowing disorders.
o Provides evaluation of speech and language abilities through diagnostic testing establishes goals and estimates prognosis for recovering.
o Records and reports the client's progress in treatment and any change in patient's condition to the physician.
o Instructs family member(s) and members of the health care team in methods of assisting the patient in improving communication skills.
o Re-evaluates patient's progress and the established rehabilitation program as often as necessary.
o Prepares periodic reports, as required by the Agency, which are sent to the physician and third party payers regarding care being provided.
o Participates in in-service, case conferences, and staff meetings as required.
o Assist in making appropriate referral to outside audiology speech clinics or other community services as indicated.
o Assists in the development and revision of physician's Plan of Care.
o Submits assessments and progress notes in timely manner, as required by Agency policy. Provides liaison contact and follow-up with other community agencies.
Job Requirements:
o Visual/hearing ability sufficient to comprehend written/verbal communication.
o Ability to deal effectively with stress.
o Must be able to meet the physical demands of providing speech therapy.
